Many young people with mental and behavioral health problems leave state custody at age 18 without a high school diploma or a job. They experience loneliness, depression, unemployment, homelessness and crime at a greater rate than other young adults do.
Transition Services reaches young people before they age out of custody. It provides support, guidance and advocacy for individuals ages 17-24 who need help preparing for life on their own.
Transition Services links young people to the full range of community health and supportive services that provide:
• Career consultation, job placement and coaching
• Education assessment, planning, enrollment and support
• Social and independent living skills training
• Subsidized housing
• Physicians and medication services
Someone is always there
Young people have trusted adults to talk to when fears and disappointments arise. Someone is available 24 hours a day and seven days a week if a crisis occurs.
The ultimate goal is for young people to graduate from Transition Services and achieve independence and success personally and professionally.
